Before you apply
We would like to invite you to learn a little more about our agency. The following documents will give you more insight into Lifeline’s agency policies and practices concerning international adoption. These policies and practices will be discussed with you after you apply to Lifeline. You can also view a sample adoption services contract which is very similar to the one that you will be asked to sign after you apply.

Sample Adoption Services Contract
This is a sample contract essentially similar to the one you will be asked to sign at the beginning of your adoption process.
What you need…
If you are interested in applying for any of our International Adoption programs, please complete and send us the Doctrinal Statement/Christian Questionnaire, Adoption Application, and application fee of $250. We will contact you within 5 business days of receiving your completed initial paperwork. Please contact us at 205.967.0811 or dave.wood@lifelineadoption.org for further information.
Each of the PDF documents below can be accessed using Adobe Reader. Once the forms have been completed, simply return them to Lifeline. There are three ways to submit your application:
1. Scan the documents and email them to applications@lifelinechild.org.
2. Fax the documents to 205-969-2137.
3. Mail the documents to:

You may pay the necessary application fees online through our payment portal. If you are not paying online, you may mail a check to Lifeline for the necessary application fees.
Doctrinal Statement/Christian Questionnaire
This form discusses our Christian standpoint as an agency and is required to be completed as part of our application to adopt.

Passport Application
You will need to apply for a U.S. passport if you do not currently have a valid one. If you have lost your passport or it has been stolen you will need to complete a DS-64 form as well to turn in with your passport application.
