What You Need
If you are interested in applying for our domestic adoption program, please fill out and send us the Doctrinal Statement and Fee Explanation Sheet in the Information Packet, the Christian Questionnaire, and the Application along with your application fee. Additional information will be provided to you to access other documents once you have entered into the program.
Each of the PDF documents below can be accessed using Adobe Reader. Once the forms have been completed, simply return them to Lifeline. There are three ways to submit your application:
1. Scan the documents and email them to applications@lifelinechild.org.
2. Fax the documents to 205-969-2137.
3. Mail the documents to:

You may pay the necessary application fees online through our payment portal. If you are not paying online, you may mail a check to Lifeline for the necessary application fees.
Domestic Info Packet Insert
This packet is an introductory packet that will introduce you to our Domestic Adoption program. Please follow the enclosed instructions on completing necessary forms and returning them to our office.
Doctrinal Statement & Christian Questionnaire
This form discusses our Christian standpoint as an agency and will be required to be completed before an application can be sent. Both husband & wife must complete a separate form.
Application Packet
After we have received the forms enclosed with the Information Packet, the Application will need to be completed and returned to Lifeline Children’s Services.

*Note: You will also be required to fill out various other forms that will be provided by Lifeline, including Child Abuse and Neglect forms and Fingerprint cards.
